There is no coverage for pre-existing medical conditions that existed within 180 days prior to the effective date of the policy.
Health services:
- Emergency medical, including hospital and physician services
- Extended healthcare, including private duty nursing and medical appliances
- Healthcare practitioner services
- Prescription drugs
Dental services:
- Emergency dental treatment and relief of dental pain
Transportation and accommodations:
- Ambulance transportation
- Transportation services to bring you home in a medical emergency
- Expenses to return home children under your care
- Expenses to bring someone to your bedside
- Hotel and meal expenses
- Expenses related to your death
Accidental death and dismemberment:
- Up to $25,000 if you die, become completely and permanently blind in both eyes or if 2 of your limbs are fully severed within 365 days of the accident
- Up to $12,500 if you become permanently blind in 1 eye or if 1 of your limbs is fully severed within 365 days of the accident
